My only answer to your second question would be that perhaps the prostitutes in question are ignorant of the risks. Even though they are sex workers not all of them are sex experts.

In anal sex, the rectum is likely to develop tiny tears and cuts because it is tight and the muscles inside it push outwards while a penis pushes inwards. Both vaginas and mouths can easily have little tears or scratches inside them. How often do you bite your mouth while eating, get an ulcer, or scratch your mouth by eating something kind of sharp? For that reason anal sex probably has the highest risk, and vaginal and oral sex carry about the same risk.
HIV/AIDS can be transmitted before ejaculation, as the pre-ejaculate (the small amount of fluid that comes out of a penis of a sexually aroused male, before he ejaculates) of an HIV-positive man carries the virus. Therefore the "withdrawal method" is not going to help.
Any prostitute that offers any condom-free service should be avoided. By having sex with that prostitute, one is effectively having sex with all his/her previous clients too.
